Application Procedures

1. Application form can be obtained through the following ways:

• in person / by proxy at the Recognition of Prior Learning (ICT (O&S)) Assessment Office (“The Office”)
• download form from the website of the Office

2. Submit the following documents:

(i) Application form
(ii) Application fee (crossed cheque made payable to the “HKU SPACE”)
(iii) Employer/Referee Review form
(iv) Supporting documents of the working and relevant experience
(v) Photocopy of HKID card
(if applicants submit their application to HKU SPACE Enrolment Counters in person, please bring along the original copies of the items (iv) and (v) for verification of documents).
